Description:

Illiana-Danville VA Medical Center has teamed up with AgeCentral to offer Veteran Directed Care (VDC). VDC provides assistance to veterans to help them remain in their homes and is a more flexible option than traditional in-home services.

A trained options counselor helps the veteran determine what kind of help is needed, and then VA funds are used to-
*Hire and manage an in-home assistant, including hiring a family member or friend to help with personal care and assistance around the house
*Make the home more accessible
*Order meal delivery services

Hours:

Office hours: Monday through Friday, 8 a.m. to 4 p.m.

Intake Process:

The veteran must contact their primary care social worker at the Bob Michel VA Outpatient Clinic for an official referral into the program.
