Description:
Oak Street Health's Peoria Clinic is a primary care center for adults on Medicare and is the only primary care provider to carry the AARP name. Oak Street Health also helps patients maximize their Medicare coverage and coordinate other healthcare services such as specialist appointments and social services. Care teams develop personalized preventive care plans to help patients achieve their physical and mental health goals. Services offered at the Peoria location include primary care, chronic condition care (especially focused on those ages 65+), help navigating Medicare benefits, wellness and preventive care, and immunizations (Flu, COVID-19, and more).
Oak Street Health's Peoria Clinic also serves as a cooling center during business hours for those needing a break from the heat.
