Advocates for Access is a center for independent living, empowering and providing services to people living with disabilities in Peoria, Tazewell, Woodford, and Fulton counties.

Please note that for more complex durable medical equipment (DME) such as Hospital Beds, Lift Chairs etc., Advocates for Access connects customers who need them to those who have them to give and/or provides the guidance to get one from their provider. The same goes for blind mobility aids and related items.

In general, anyone with a disability. Please see below for specifics on certain programs.
*Self-advocacy support (instead of case management): Situations where an individual with a disability is encountering trouble in accessing services or needs guidance in navigating the social service or benefits system
*Statewide Referral Network (SRN) housing list: Anyone with a 9-digit Medicaid number
*Medicare Information/Counseling: Anyone under the age of 60 with a disability (exceptions possible for those age 60 and older who a long-term Advocates for Access customer)

Description
Advocates for Access provides housing search and information in two ways:
-Acts as a screening agent for individuals and families on Medicaid to be placed on the Statewide Referral Network (SRN) list for subsidized housing. The SRN list allows a person or family to pick several counties, even specific cities, in which they are interested in residing. When an opening becomes available, they are notified and may apply to the housing entity or complex. This is not guaranteed housing but a means to find out about available housing.
-Maintains lists of subsidized and other housing by county (Peoria, Tazewell, Woodford, and Fulton), available on the Advocates for Access website

Description
Through its community reintegration program, Advocates for Access works to assist people with disabilities in leaving nursing homes to return to community life with proper supports. The grant may be used to help fund security deposits and first monthâs rent, as well as provide furniture, assistive technology, household items, and groceries.

Eligibility
Criteria includes:
-Applicants should currently reside in a facility licensed by the Illinois Department of Public Health, but some exceptions apply-Must live in Illinois
-Must be a U.S. citizen or legal immigrant
-Must have a severe disability that will last for 12 months or longer
-Cannot have more than $17,500 in non-exempt assets
-Must be receiving or have applied for Medicaid benefits

Advocates for Access offers Ready to Roll, a summer ramp construction program in conjunction with His Helping Hands and ADA Total Access. Advocates also provides applications for a separate ramp construction (and home modifications) through the City of Peoria

Description
Illinois Telecommunication Access Corporation (ITAC) administers a program to ensure equal access to communication for the Deaf and Hard of Hearing community. Through the use of a voucher program, anyone who is Deaf or Hard of Hearing can obtain a free TTY, CapTel or amplified phone. Advocates for Access serves as a selection site for these phones.
