All vaccinations are by appointment only. Find vaccination locations on the map and get appointment information by following the listed links.

Please bring documentation to your appointment to show that you are eligible for vaccine as part of a priority group (find your priority group here: https://coronavirus.illinois.gov/s/).

Examples of acceptable documents include:
-Personal identification (State ID or driver!€™s license, US or foreign passport, Consular ID, Temporary Visitor Driver License)
-Employee identification (work identification or badge, paycheck stub, CityKey card, letter from employer)

Providers may NOT ask individuals to provide documentation regarding their immigration status.

As the federal government distributes additional vaccine doses, additional appointments will become available. New sites and pharmacies are being added daily. Please check back regularly for available appointments.
