Description:
Home Care Ombudsmen are located throughout the state of Illinois and work to protect the rights of older persons and adults with disabilities living in the community who:
are enrolled in the Medicare Medicaid Alignment Initiative (MMAI) and receive services from a managed care organization (MCO); or
receive services from one of the following Home and Community Based Services (HCBS) Waiver Programs:
-Persons who are Elderly
-Persons with Disabilities
-Persons with Brain Injury
-Person with HIV or AIDS

A Home Care Ombudsman can help you by answering questions you may have about the services you receive. A Home Care Ombudsman may also help you:
-if you have received a denial or termination letter for in-home services,
-if you are not happy with the services you are getting from your MCO or HCBS Waiver Program, and/or
-if you cannot get in touch with your MCO care coordinator or HCBS Waiver Program care coordinator.
Hours:
Monday through Friday, 8:30 a.m. to 5 p.m.

Eligibility:
Adults age 60 and above OR disabled adults between ages 18-59, and who receive services through select Illinois
Home and Community Based Waiver Services Program (HCBS), or who are Medicaid and Medicare beneficiaries and receive managed care services
